looks like the pulleys on your tensioner may have been put on the wrong way around..metal pulleys in your pic look like nu line ones..these must be bolted on from correct side to work properly...unlike the original plastic pulleys which can be bolted on from either side..remove the tensioner and try turnig pulley around.

The belt doesn't sit central normally on the lower tensioner pulley either, if you ever notice the wear on a typical plastic one.

in the pics that damo put up, its the harmonic balancer that is the issue.
you can see the belt area on the balancer is out of align with the water pump.. a similar issue will happen when the tensioner is suffed see there is a gap opening where the tensioner pivots. before Attachment: belt tensioner 1.jpg [ 129.42 KiB | Viewed 201 times ] after Attachment: belt tensioner 2.jpg [ 83.81 KiB | Viewed 203 times ] oh and the metal tensioner pulleys only go on one way, they are slightly offset if you try put them on the wrong way they will hit the tensioner and wont even turn.. |

my car is exhibiting the same problems . already changed my balancer but problem is still persisting.

Had the same problem after replacing a failed balancer/pulley. Was chewing through a belt a week. The pulley was still way out of alignment (by about 1 rib on the pulley). Turns out that I hadn't tightened the bolt far enough, so the pulley wasn't all the way home. Doh!
 The pulley is a tight friction fit on the crank, so it needs a lot of torque to get the pulley to fully seat and line up with the belt. Use a breaker bar, as a normal ratchet is probably not enough leverage. Mine is an automatic, so I had to take the access plate off the front of the transmission and jam a prybar in the flex plate teeth to keep the crank from turning while I tightened the crank bolt. That fixed the problem. Worth double checking.

I've got the head off at the moment, i might quickly undo the starter motor & see if i can jam something in between. i will try to tighten the HB bolt even further.
