I'd be sure it was the nuts that were wrong.
On the ones I got from Tempe, the thread on the studs on the adapters are different to the thread on the ford hubs, but seem to be same size. Tempe provided two different sets of nuts because of this.

rattle guns are a good way to warp the brake discs and cause brake shudder problems, because they often over tighten the wheel nuts unevenly... the only time a rattle gun should EVER be used on a wheel is to undo it, or when using a special "torque" stick socket.

Hey djmikey, I had a similar thing happen. Except my wheels had been on for close to a year, and without hub adapters!

impact gun or not you still didn't tell us how many turn you got on the nuts.

by the looks of your pics you barely had 3 or 4 threads holding on

The threaded part of the studs are about 3cm long, so you can't say there was only a few turns holding them on. To cut a long story short, I am nor returning the adapters and selling the wheels.

Below is a pic of my car with the wheels on. (taken b4 my drive)

Your selling it as you did a mistake and won't admit it? s**t happens dude, you fitted them wrongly and it came off. It has nothing to do with the hub adapters as you can see a number of people used/using them and have no problems except you.

did you check or did you just assume 3cm long is enough.

have you seen how long the factory mag wheel studs are on falcons. the difference is huge, when i put EF mont ghia rims on my dads EB i changed all the studs to avoid your problem

hub adapters aren't the problem, i've run 2inch wide adapters/spacers on my hilux 4x4, it also has 35inch tall, 11.5inch wide tyres on 15x10 inch wheels.

my hilux puts a hell of lot more stress on the studs and nuts while offroad then you falcon was over going too, and i have never once had anything come loose or fall off.
