I have asked this same question, and everyone seems to use either sikaflex or marine silicone. I cant get my head around the hollow backing either, so they must just use massive dolops (spelling) of it.

Try just putting the silicon on the narrow edges which are the only portion to actually touch the door trim. No wonder they're secured with clips ex factory lol.

Try Aquarium grade Silicon Matt. I have used it and it works well, wipe down all your surfaces with Metho or Acetone before applying. Use resonably sized blobs where the old plastic pins were, silicon will go through the existing holes in the door trim and help hold the spear.

Yep, I used Marine Silicon to put my spears on my door trims. And yes, big blobs of silicon did the trick.
Use some masking tape to hold them in place while it goes off. BenJ
